Georgia Dispensing Optician Application Help

Need some help please with my Georgia Dispensing Optician Application. I have my ABO, NCLE and an Arizona State License. I have had my Arizona License since 1999. I have already requested the Arizona Board to forward the Certificate of Other State Licensure Form to the Georgia Board. I think I should apply based on experience, however, the application requires an Affidavit of Experience and Employment form be completed by a supervising OD. As a licensed Optician in Arizona I do not practice under an Optometrist supervision. How would I complete the Affidavit form? Also, would I need to register under the Georgia Apprenticeship program?
