There was a thread a while back on the Shamir Duo no-line bifocal lens.... Since it's been over a year, can anyone update on how people seem to like it? We just started working with a lab that offers it. Any feedback would be greatly appreciated.

National Optronics 4Ti Tracer

Hi! I (foolishly, I think!) bought a National Optronics 4Ti tracer. My goal is to get the OMA trace files for my own nefarious purposes.

it seems this particular unit was made for Walmart, and only works with their software. That suggests it's a doorstop.

Does anybody know a workaround, or have any technical literature on this machine that might help me get at the data?

Thanks!

Vision and Parkinson’s Disease

Vision and Parkinson’s Disease

As people with Parkinson’s know all too well, the disease can cause many more symptoms than those that are noticeable from the outside. Some of the lesser recognized ones — even among doctors — involve the eyes and visual system. Parkinson’s can cause many symptoms, ranging from dry eyes to double vision. Not only can visual disturbances interfere with reading or driving, they can worsen walking or balance problems, and even contribute to hallucinations.

Vision problems can be due to Parkinson’s disease (PD), the medications used to treat it, or to unrelated conditions of the eye or eyelid.

Visual Disturbances Can Be Part of Parkinson’s Disease

Just as Parkinson’s can affect general mobility, it can impair movement of the eyes, which may result in difficulty keeping objects in focus or seeing things close up, as when reading. To treat these symptoms, doctors may prescribe corrective lenses and/or prisms (special lenses) and adjust Parkinson’s medications.

Parkinson’s may also dampen the ability to sense individual colors or make them appear duller. PD causes a loss of retinal cells in the eye that rely on dopamine to process and perceive color.

Parkinson’s may also impact the eyelids. People with PD blink less frequently, which can lead to dryness, irritation or burning of the eyes. Sometimes it even causes blurred vision. Eye drops and ointments may help.

Some people with Parkinson’s have the opposite problem of excessive blinking which may lead to involuntary closure of the eyes, called blepharospasm. The treatment of choice for this problem is botulinum toxin injections into the eye muscles.
A few people experience a syndrome called eyelid apraxia that causes difficulty opening the eyelids. Special tape, glasses with wire loops or “crutches” can help hold the eyelids open.

Many people are familiar with dyskinesia — the abnormal, involuntary movements of the body that may occur with longer course of disease and many years of levodopa usage. Few people are aware, though, that dyskinesia can also affect the eyes and cause blurred vision. If someone is experiencing dyskinesia in other parts of the body, this might be the culprit for visual disturbances. Medication adjustments may be the solution.

While dyskinesia typically happens in the “on” state — when Parkinson’s symptoms are otherwise well-controlled, some people experience worsening of vision as medication starts to wear off and Parkinson’s symptoms return. Medication adjustments would likely be beneficial in these situations as well.

Medications May Affect the Eyes

All prescriptions have potential side effects and some of these involve the visual system.

Anticholinergic medications, such as Artane (trihexyphenidyl) — which is used to treat tremor — can lead to dry eyes and blurred vision. Treatments include eye ointments or drops, or warm compresses. If symptoms are severe, it may be necessary to switch medications.
In more advanced PD, hallucinations (seeing things that aren’t there) and illusions (misinterpreting things that are there — thinking a tree is a person, for example) may occur. These visual symptoms can also be a side effect of some Parkinson’s medications. In general, they are more likely to happen in people who have ongoing problems with their vision (they need corrective lenses or have other conditions affecting the eyes, for instance).

Visual Problems Are Also a Common Part of Aging

As you get older, several diseases of the eyes can occur regardless of whether you have Parkinson’s disease. These include cataracts, macular degeneration and glaucoma.

A cataract is a clouding of the lens of the eye, which can cause blurred or double vision, color fading or poor night vision. Surgery can correct this problem.

Macular degeneration is damage to the macula — a small part of the retina that maintains the center field of vision. As this disease progresses, images in this area can appear dark, blurry or distorted. Limited treatments are available.
Glaucoma is an elevated pressure in the eye, which can damage the optic nerve and lead to vision loss. It is treated with eye drops or surgery.

Many Ways to Manage Vision Problems in Parkinson’s

When a visual symptom arises in the course of Parkinson’s disease, it’s important to rule out other conditions. Don’t assume it’s due to PD, especially if it comes on suddenly or fluctuates. People with Parkinson’s can certainly have other medical problems that can affect vision.
If Parkinson’s or the medications used to treat it are thought to be the reason for your visual symptoms, your movement disorders specialist can adjust your treatment regimen. If this doesn’t work (or even if it does), consultation with an eye doctor or a neuro-ophthalmologist — a doctor with training in both neurology and ophthalmology — may be beneficial. He or she can examine for and treat all of the conditions listed above.

If you’re experiencing changes in your sight, don’t automatically blame it on age or Parkinson’s. Raise the issue with your doctor, ask what could be causing your symptoms and determine what can be done to ease them. Together you can work to find a solution that will lessen your visual symptoms and improve your quality of life.

Lab Equipment Questions

What surfacing equipment does everyone use?

We use Gerber's Step One/SGX/finer/polisher system which was bought new in 1999. The step one and polisher have given us some problems over the past few years, but the Doc said all this stuff is paid for and he'd like to keep it that way for as long as he could. I guess paying for a tech to come out if need be is cheaper than spending a quarter million dollars on a whole new lab! We currently only surface CR-39, but have a Kappa tracer/edger for finishing.

If it came down to replacing our lab equipment, what would be a suitable replacement?

Distometer is lined up at the 2mm mark

Hi all,

The distometer I just started using is lined up at the 2mm mark when there is no pressure on the plunger. Should I subtract 2mm from the reading, or is the reading accurate?

It looks exactly like the distometer in this picture: http://ift.tt/1kbPxje

Need help with glasses

My patient (48 years old) wore -5.25 -1.50 x180 /-6.75-1.00x180 with +1.75 rx for two years. She never had success with computer vision, so she wore a reading only lense we made last year. This year she was prescribed -5.00-1.00x176/-6.50-.50x004 with +2.00. The frame is larger b measurement, otherwise progressive type, material, bc, ext are matched. She could see great at work with these, but distance was not clear. She went to an different doctor who gave a new rx of -5.50-1.50x175/-7.00-.50x005 with a +2.25 add. Now she said she can see great distance and near. Was traveling the last week and vision was great. Today she's back at work and she's getting a headache. She says after being on the computer and getting up to walk around she feels dizzy, motion, and seems to have to re-focus. The vision is clear up close though. I used same frame, same lenses, same lab. I called my lab to see if they could see any manufacturing differences and they said there are none. I am stumped. Any ideas?

Best Tintable Polycarbonate Stock lens???

I've been getting a lot of folks lately wanting light brown gradient lenses, usually in Drillmount Cartiers. They've been really picky about their tint color. All the lenses i've tried have come out either off-color or with sharp lines even after a quick dip in the dye or the neutralizer.

Anyone found anything that worked well color-wise?

How about Hydrophobicity and Hydrophilicity on all lenses ?

So far Hydrophobicity is only used on AR coated lenses, but not on plain regular lenses of all or any type.

A hydrophobic top coating (or the many other names as oliophobic, ect.) on AR coated lenses fills the gaps, or valleys between the under a microscope, mountainous looking surface, creates a smooth and slick surface that is easy to clean.

Most optical retail professionals do not know that this is a familiar product in more industrial areas than just the optical. It is used from hospital operating rooms to building walls to make them waterproof.

There is an excellent description on these coatings in the Paint&Coatings Magazine published 9 years ago.

-3.75 -1.25 x 005
-3.75 -1.25 x 180 add 1.75 sg 24
Autograph III Fixed 18 1.67 w/Crizal Alize in a Prada VPR 180 ophthalmic 52/18/135

now I'm kind of stumped to why that is because in her old rx she said she doesn't get these headaches. Only thing that comes to mind is the Progressive is different (variable/fixed, lens material, and add power being off a quarter diopter). Can anyone shed some light or help me out on why she gets the headaches?

Post on another thread got me to thinking---

Do any frame companies buy back their inventory if you want to drop the line?

I'd guess there's got to be some but frankly I'm not aware of any.

Are you?

Binculars focus better than glasses...these glasses are wrong!!!

Patient is complaining that new glasses are not focusing through the left eye the way they do when he looks through binoculars left eye piece. I assume it's aperture and magnification systems being different. Am I right?

Right -.25 -.25 x 150
Left -.25 -.75 x 180

old rx 2013
right -.25 -.50 x 135
left -.25 -.50 x 180

Help me decipher Doc's notes:

RX shows no real change without improvement
SL shows MUILD-MOD
CN Changes OS>OD causing complaint

I'm afraid he's the type that no explanation will be sufficient however. After all, what do OD's know about correcting vision that the patient doesn't?

Medicare DME supplier question

We are an optician owned optical office signing up to be a medicare dme supplier. We have a part time optometrist who works 2.5 days per week. One of the medicare supplier standards states that a dme supplier cannot share a practice location with another medicare provider. However other medicare opticians have OD's in their office. Our OD does not pay toward overhead, he's an independent contractor. Does anyone have any insight into this rule and how are other offices able to do this?

The new long mystery specialists

The new long mystery specialists will be discharged worldwide on October 26 in London. This is the first run through in history that the film will open on that night in silver screens around the UK and Ireland, giving general society the chance to be the first on the planet to see the very foreseen continuation.

My afternoon chuckle.

Afternoon everybody,

So, had a patient come in today with two sets of glasses from Warby Parker. She explained to me that she was having some severe eyestrain from her new glasses and was obviously concerned. She's a career office worker and cannot even make it two hours in these glasses without a migraine. So I went to work.

Her Rx as written,
OD +.25 -2.75x 180
OS +.50 -1.25x 065
The glasses received,
OD +.25 -2.75 x 002
OS +.50 -1.25 x 068

Not bad, they were paying attention there. But this is where the correct ends. The patient opted to take her pd with the credit card to the face method and received a PD of 59, the glasses however are at 64. I marked them with the lensometer and found the OS's center to be 2mm higher than OD. There are numerous other defects I found but mostly unimportant at the time (stress marks in the lens, wavy AR coating).

So, after a gentle discussion of the perils of online sellers she agrees to get new lenses made for her frames. I find out that a bout of Bells Polsey as a child left her right side underdeveloped and as such I find her Pd to actually be OD 26.5/ OS 29.5 (OU 56). To be brunt she was pretty embarrassed over the incident. I do believe I've caught another lifetime customer here. ^_^
My afternoon chuckle.

Need Some Help Here!

My husband made 2 pair of glasses for our 3 year old grand son. We live in Northern NY - he lives in Scotch Plains NJ. Is there anyone within 10-15 miles of Scotch Plains that could adjust them? Please PM me with address etc. I would certainly reciprocate in the future! :help:

Thanks in advance!

The AOA and Opternative

Riddle me this:

57 year old woman R +0.50 +1.75 @ 20 L +1.00 +1.50 @170 Been wearing Varilux CR-39 for the best part of 20 years. Switches dispensers and receives a pair of Kodak Unique progressives in 1.74, no AR coat.

B&M dispenser (or should I say optical boutique salesperson?) cannot understand why the patient sees rainbows around everything. Asks patient if the lenses had polished edges (no).

Oh, and it took 5 weeks to get them!

(And the price she paid! Oy vey.)

Full disclosure: patient is my sister living in Central Missouri.

This is a classic reason why some ordinary folks switch to on-liners. I priced this job out, the frame would have been ok, that was fairly priced, but the onliners (and I checked several) were all in the 2.5 franklin zone, where at this boutique, she was charged 5.5 franklins.

There was no discussion with patient as to why the material change was made, nor any discussion about the pluses and minuses of the new material. Her script has been stable for the better part of 8 years, this was a new pair because the old ones were getting just a bit beat up after 4 years.

There has to be an understanding that at least in part, the brick and mortar dispensing businesses are at least partially responsible for pushing folks towards internet purchasing.

My sister did have the good grace to be embarrassed when calling me for help to figure out what is wrong with the lenses and apologized profusely for not checking with me prior to making the purchase. She's going in on Tuesday (they live 60+ miles from the nearest dispenser) for a full refund.

I appreciate you registering my comment about Eyeglass Rule, 16 CFR Part 456, Project No. R511996. Recently I attempted to buy a second pair of reading glasses. Though I had received my eyeglass prescription only 16 months prior, and my prescription hadn't changed, I was forced to pay for a costly and inconvenient eye exam. I require eyeglasses to read, but my prescription rarely changes. The current structure is arbitrary, expensive, and seems unnecessary. Please allow us to use a prescription beyond a single year. Thank you.
The zeroes. (They're all obviously fake, BTW)

Initiative Name: 16 CFR Part 456 ; Agency Information Collection Activities: Review; Comment Request Ophthalmic Practice Rule (Eyeglass Rule): FTC Project No. R511996

This proposed rule change proposed by Senator Charles Schumer is purely at the behest of on-line retailers of eyeglasses. They are finding it difficult to forward their business model because most prescribers do not measure the inter pupillary distance as a routine part of writing the prescription. This is usually done by the optician when glasses are being fitted. The cost of performing this service, usually done by specialized equipment, is recouped at the sale of the glasses. Now the big retailers are trying to get this information about inter pupillary distance for free and then also take the profit from the smaller and usually individual glasses provider. Of course when there is a problem with the online glasses, the online retailer will be no where to be found. Thus, problem solving will fall to the prescriber. The patient will assume that the mistake is in the prescription and demand that the writer of the RX fix the problem for free. Finally if an individual is being encouraged to go online to buy a bifocal that is a progressive, that is fraught with potential problems. There are SO many variables about fitting and adjusting glasses to make a patient happy that there is no way this can be done properly online. So if you mandate this you will likely make some people very unhappy because they will not be fit properly. I, for one, will tell them that, unlike if they buy the glasses from me, they will have no recourse for fixing their problem. I will not let Big Online Retialers steal my profits and then solve their problems too.

MD's rechecking IOP's: Discuss

I've worked as an eyecare professional for ten years with some of the top glaucoma docs in practice, one of whom actually wrote "The Glaucoma Book".
Recently I started working with a new ophthalmologist who re-checks my IOP's (many minutes later) and after every patient comes back to tell me that she finds the IOP SEVERAL points lower, every single time.
I asked her specifically if she is putting more fluro in the eye before she checks...she says no.

My point: She is not putting more fluro in when she re-checks IOP, and so...the mires are thinner and therefore, the IOP would SEEM lower.

I'm confident in my abilities and do not doubt my measurements.

I find this annoying and more. But I'd like to hear some other opinions.

Is this happening to you? How do you deal?
Am I over reacting?

Optronics 3B Blocker Blackout

I recently started working with an Optronics 3B blocker and have noticed difficulties blocking high minus and polarized lenses. For some reason the layout grid seems to blackout with polarized lenses similar to when polarized lenses are 90 degrees apart. With high minus lenses only the center lensometer mark is visible with the outer pin marks non visible. Is there an adjust I'm missing to be able to see lensometer marks, seg or progressive marking on polarized lenses?

Essilor Reports 14.5% Jump In Q3 Revenue; Backs Full-year Goals
Today

(RTTNews.com) - French eye-care firm Essilor International SA (ESLOF.PK, ESLOY.PK) Thursday said its third-quarter consolidated revenue climbed 14.5 percent to 1.620 billion euros from last year's 1.415 billion euros.

The increase included an 8 percent positive currency effect, mainly from the rise in the US dollar against the euro, and a 1.7 percent increase from changes in the scope of consolidation. On a like-for-like basis, revenue grew 4.8 percent.

The company witnessed sustained robust growth in North America and Europe, led by the stepped-up consumer marketing campaigns.

Faster growth of 9.9 percent was seen in the Sunglasses & Readers division, the biggest gain since the division was created in 2010.

The company backed its full-year goal of revenue growth between 8 and 11 percent, excluding the currency effect, and in excess of 4.5 percent on a like-for-like basis.

Essilor still expects a contribution from operations equal to at least 18.8 percent of revenue.

How much flatterare aspheric lenses?

Hi, this is my first post on the board.
I am looking for a guide to how much thinner a typical 1.53 or 1.6 index AS SV plus lens is than standard CR39. This is to help dispensers I work with who have very little experience in this area.
I can appreciate it can only be calculated if I know several proprietary variables and exact blank size, but I wonder if there is some kind of rule of thumb? Something like the approximate sag formula but for plate height in an aspheric?
Thanks,
Jonathan
