Is an intelligent blocker (like ICE 9000) much more efficient than manual blocker?

We have been doing our blocking with an old manual AIT blocker. It takes a little time and there are a certain number of errors but it seems to work. Recently my optician has been asking about acquiring a Nidek ICE 9000 intelligent blocker. Is this far preferable to the manual blocker? Faster? More accurate? Most of our problems seems to come when we block bifocals (they don't always come out as straight as we wanted). Also, can these blockers be directly hooked up to Santinelli 7070 and 9000SX edgers? Does the direct hookup provide increased efficiency and accuracy?

100,000+ cuts on a used Santinelli LE9000SX PLB?

A used Santinelli LE9000SX has become available from a colleague in my area. It has about 100K cuts but apparently is working fine (I didn't test it personally). The cost of the unit would be $1500. On the other hand I have a Santineli 7070 which works but is making a loud noise and probably requires a new spindle and perhaps some other refurbishments. Just wondering if purchasing an edger with this many cuts is worth doing? We do under 8-10 jobs a day.

Checking Distance Optical Center on FT

I ordered a pair of FT 7x35 that with a distance PD of 65 and OC 5 mm above the line. I keep getting the lenses back with the OC's far greater than 65 in the distance portion of the lens. My question is do you check Flat Tops in the distance lens like you would a SV? Do you mark the OC on both lenses in the distance and they should read 65 mm apart and 5 mm above. Or am I missing something. I sent lenses back to lab and they told me they are correct. The distance OC reads 5 mm above but about 75 mm apart. Wouldn't that induce prism for the patient? Any help would be great. Also, I cant find any material on the proper way to verify flat tops so any literature would be helpful also. Thanks!

I am in the process of opening an office in Georgia. In order to finish my credentialing with VSP I have to fill out my average costs and prices for lenses. I obviously don't have an average cost of lenses because I have not begun ordering any and I have not gotten set up with all of my labs. Is this in order to set up our reimbursement schedule? So, this happened the other day I wonder how you might handle the same situation.
We have an office policy which states ~ Purchase one complete pair of glasses get the second complete pair of glasses at half price. ~ We accept this as canon in the office among other rules and regulations.
We are able to offer this discount because the lab we use extends to us a similar discount that we in turn extend to the patient. If we purchase 2 pair of lenses for the same patient (some restrictions apply) we get 50% off the order if it’s for the same patient. This is an important point as you will see later.
A patient came in and purchased a complete pair of glasses out of pocket with no insurance benefit.She then wanted to purchase lenses for her sun-glass frames that she purchased in Spain.She asked if we had any deals or specials that we could offer for this second purchase. I told her that with a second purchase of both frame and lens we could offer her 50% off of the second pair. She did not want to purchase a second frame because she already had a very nice frame that she purchased elsewhere.I told her the price of the second pair of lenses without the purchase of the frame.She did not want to spend that amount on the lenses themselves but asked if we could extend the curtest of a second pair at 50% off for just the lenses.I told her that although I neither had the power nor authority myself to make that decision but I would gladly ask the assistant manager. She then noted that if we could not reduce the price for the second pair then she was not interested in purchasing them.
Well, sais I. An interesting quandary.
Here’s the ~IF, THEN~ statement. IF the manager approves the sale and extends the 50% off courtesy to the customer, which is outside of the rules set in place, THEN we capture extra revenue AND the customer has a positive experience. If not, then the rules have been preserved and the environment that we work in remains sterile with the sanctity and purity of our beloved dogma intact.
Ill tell you what happened and then pose my question. I thought to myself ~Self, here is an opportunity to make more money for our master! We cants ever have too much money can we precious, no we cants! I told the customer I would gladly champion the cause to the assistant manager. As I was speaking to the assistant manager about the situation, she was shaking her head no, until I finally conveyed the full message and then she barked NO and was quick to point out that the customer did not purchase a frame. I quickly reminded her that the lab offers us a discount for the second pair regardless of a frame purchase and we would capture additional revenue from the sale if this was approved. She said WELL I’LL HAVE TO THINK ABOUT IT. I told the customer I would let them know that the management’s decision was regarding the second pair in the afternoon. Later that day when I asked what her (the assistant managers) decision was, the assistant manager told me that we had already made three “deals” this week and we can’t keep doing things like this but she did approve it.
The questions then are ~ Do you bend the rules when it is advantageous to capture additional revenue? Should employees be encouraged to find ways like this to create more sales? Should employees be discouraged from rule bending? Should employees question managements decision once spoken when the result ends in both profits and customer satisfaction? Should this employee been given positive feed back from management for dynamic, outside of the box thinking that generated more revenue for that sale?
In addendum to this situation, the very next sale was a similar situation where the patient had flex money to spend and needed a second pair of lenses for a frame that she already purchased from us. The 50% discount was not offered in this situation and was not asked for by the patient. The patient was satisfied that her flex spending money was used before it was lost and all was well.
A second set of questions arise from this pertaining to both situations: Is this a model of thought that employees should be aware of? Is it unscrupulous to ask management for a discount for one patient if they request it , but not make the second patient aware of the discount for the same situation?

Feb 19 Essilor, the world's largest maker of ophthalmic lenses, said on Friday that operating profit rose 19.6 percent in 2015 and forecast that revenue would grow over the coming years, helped by demand from an ageing population.

Operating profit rose to 1.18 billion euros ($1.31 billion) last year from 989 million in 2014, while revenue rose 18.4 percent to 6.72 billion.

"In 2015, the optical market continued to bedriven by demand from the world's ageing populations and unmet vision needs," the company said in a statement.

Essilor forecast full-year 2016 revenue growth, at constant exchange rates, of above 8 percent, including a like-for-like gain of around 5 percent.

I need help

My patient has a frame from 2014.

The original rx was : +6.25+1.25x063/(bal)+6.00+1.25x117 +2.25 add
The new rx is : +6.25+1.25x060/+6.50+.75x080 +2.25.

The new and old lenses are same exact brand, HI 1.67, both are 8 base.

The problem is that the patient is complaining that the frame is now too tight since I put the new lenses in. Even more curious is that in 2015 I did a scratch warranty on the original lenses and we had no issues.

Any ideas?

Blind people wearing adidas.

Adidas has a pretty cool sport sunwear line, IMO.

If your Rx is not too bad, there is "direct glaze".
If you can't "direct glaze" because of the frame, they have "adapters" which are semi-rimless mountings that jam into the chassis.
If you're blinder yet, they have "clip ins", rimless and full rim.

The rimless clip-in I have not seen.
The full rim clip-in I have a sample of. It has an eyewire attached to a silicone strap nosepad that attaches to the frame. It's very slick and seems to work great in the demo frame.

I looked under the slit lamp at how an eyewire could attach to a silicone "nosestrap". It looks like some kind of clear adhesive. Looks like it's robustly glued on there, all right.

My question to any who may have used this device: does the silicone "nosestrap" + clip-in eyewire separate when the silicone degrades? To me, it may be a sunshine and unicorns kind of design.

I hope I'm over-worrying.

Prescribed special instructions

Tried searching for this topic but couldn't find anything related to this situation, so hopefully I can get some help and not ask something already addressed.

We recently had a patient take his Rx with him and purchase glasses at a big box location. It happens, not a huge deal usually. But in this case it turned out to be a pretty big deal because the patient has a balance lens in his OS due to loss of vision and the Dr noted on the Rx that the patient has to be in Poly or Trivex (as is typical for safety reasons). The place this patient went to did not fill the Rx as prescribed and when the patient brought his glasses into our office for an adjustment it was noticed.

We explained this to the patient, he was unaware of being put in CR-39 and told us he had purchased his glasses during a sale the location was having (he didn't say this but I assume it was one of those "2 pair for $39" type of deals.

It had been explained to the patient during his exam why he needed a safety material, it was noted on the Rx that he walked with, but the location that he had fill the Rx did not follow the instructions. This is the only one that we know of happening of course.

Question is; is there an organization that can be contacted about this? I imagine legally the advice will be "Tell the location to fix it. Since nothing happened to the patient as far as damages go there is nothing that can be done in court." But it seems to me the AOA or the state could be compelled to audit a location to verify if outside Rx's are being filled as the Dr prescribed them. Maybe children aren't getting safety lenses that Drs prescribe, or poly non-adapts are being forced to wear material that isn't optically "safe" for them.

Add OD to Ophthalmology Practice???

Hello! Does anyone have experience with employing an OD at an established Ophthalmology practice? I run the optical at one such facility and we do not take any vision plans. I would like to begin taking insurance to increase our traffic flow, but the Ophthalmologists are leery of doing this for fear it will interfere with their reimbursement rates from Medical Insurance. I have suggested hiring an OD, but there concern is patients with vision insurance wont want to see them or use their medical insurance for their exams anymore and we will lose out on the medical reimbursement rates as well as the refraction fee. Any advice??? Thank you!

More Intermediate for Office Work

so I've had two patients who are first time progressive wearers that use their computer heavily for work. One does taxes and the other is an Insurance Coordinator.

I've put them in the same progressive (Varilux Physio). However both of them seem to have trouble seeing their computer. I tried adjusting it to see if maybe they were looking through the transition between distance and intermediate, but that didn't work. I want to switch them to a Unity PLx19, here are there rxs

patient 1:

OD +1.50
OS +1.75 -0.25 X 145 Add 2.00 sg ht 24. PD 58
frame is a Clearvision\BCBG, Mila, 52/16/135

patient 2:

OD +1.25
OS +3.00 -1.00 X 010 Add 1.75 sg ht 23 PD 30.5OU

frame is a Silhouette 58/21/145.
